Falls Prevention in Older Adults Assessment and Management

This is a Multifactorial Falls risk assessment, to determine the cause(s) of your falls in order to prevent them from occurring again. This involves checking medical history, medications, blood pressure check, mobility, and environmental/ hazard risk assessment

Importance of Falls Assessments

Falls assessment and prevention are critical components in my role as an occupational therapist, working with individuals at risk of falls.

 

Falls can lead to serious injuries and significantly impact a person's quality of life, especially among older adults and those with certain medical conditions

Falls Assessment Process

Here's how I, as a Qualified Occupational Therapist, approach falls assessment and prevention:

  • Falls Risk Assessment.

  • Functional Assessment.

  • Home Environment Evaluation.

  • Education and Training.

  • Assistive Devices and Equipment.

  • Individualised Exercise Programs.

  • Medication Review.

  • Collaboration with Other Healthcare Professionals.

  • Periodic Reassessment.

Falls risk assessment and prevention are ongoing processes. I periodically reassess the person's risk factors and adjust interventions as needed to address changing circumstances
